Fees and Payments

Dr. Chrisphonte does not participate in any insurance plans. Full payment is expected at the time of each appointment. Some insurance plans will reimburse some or part of the payment and information regarding treatment and diagnosis code can be provided upon request.

 

Check, cash, and all major credit cards are accepted. Receipts will be provided upon request.

Cancellation Policy/ Missed Appointments

Dr. Chrisphonte requires 24 hours notice to cancel appointments. Dr. Chrisphonte reserves the right to bill for the entire session if 24 hours notice is not given or for any missed appointment.
